Sign Up in 4 Simple Steps:

Step 1

Register your child on Hubworks:

All hubworks forms must be fully completed with all questions answered, if you do not know your login please contact Naomi in the office at [email protected]

This form allows us to ensure that as the organisation responsible for your child, we are able to act swiftly and correctly in the case of an emergency. If your child has not attended any programs with Chabad Youth or Chabad Sparks before please follow the link and enrol first at

If you have already done this, please move to step 2.



Step 2


Complete the online camp enrollment and payment form. This form manages your child's booking for the Camp IGNITE program. Bookings close on January 10th 2017 (unless booked out).

Step 3

Email a copy of your child's immunisation record to [email protected] OR Mail to Chabad Sparks, 1 A'Beckett Street S.Kilda East, VIC 3183.

This only needs to be sent once in the 2016 calendar year.

Step 4

If your child has been diagnosed with any of the following you will need to email or mail us an action plan completed by a doctor (must include a photo of your child):

  • Asthma
  • Anaphylaxis
  • Epilepsy
  • Diabetes
  • Allergies/Intolerances (food, diet, sunscreen, band aids etc).
This only needs to be sent once in the 2016 calendar year.
